JURY DUTY AND JURY SUMMONS FORMS
Culver School District pays for your time spent serving on a Jury. Per Oregon Law: "...a Juror must waive the juror's fee...if the juror is paid a wage or salary by the juror's employer for the days that the juror is required to attend court, including municipal or justice court" (ORS 10.061). You MAY still claim mileage if you qualify. We have uploaded sample forms showing how the employment questions should be answered. These samples are from 1/29/2020.
If you accidentaly receive the Juror Fee, you must notify Payroll so that the fees you received can be deducted from your paycheck per Union Contracts.

The Employee Assistance Program (EAP) provides services to help people privately resolve problems that may interfere with work, family, and other important areas of life. EAP services include counseling, work-life balance, and wellness resources.
As an OEBB member, there is no cost to you or your family members for using EAP services. In fact, anyone dependent on your income, regardless of age or where they live, and anyone living in your household, related or not, can access these services for free.
OPTIONAL OEBB OPEN ENROLLMENT 2020-2021:

INSURANCE CAP:
FOR 2020-2021 IS $1,420 PER MONTH
FOR 2019-2020 WAS $1,370 PER MONTH

A FEW NEW ITEMS/CHANGES:
1) There is a NEW SURCHARGE on DOUBLE COVERAGE with any combination of OEBB/PEBB plans within a household. OEBB will now assess a $5 monthly surcharge for each subscriber who elects to cover a dependent on their OEBB medical plan who is also covered as a subscriber under their own OEBB or PEBB medical plan. Culver will allow this Surcharge to be paid out of the monthly District CAP. If you have out of pocket expenses for Medical, this may raise your monthly deduction by $5, above and beyond any premium increases.

2) The SELECT PRICING plan rates went away this year. They were a 1 year only perk according to OEBB. PLEASE CHECK THE PREMIUM PRICE OF YOUR PLAN! YOUR PREMIUM MAY HAVE INCREASED IF YOU WERE ON SELECT PRICING LAST YEAR!
3) The price of Short Term Disability has been reduced by 50% for the 20/21 Benefit Year.
4) For the 20/21 Plan Year- Members on INCENTIVE DENTAL PLANS (Delta Dental Plans 1 and 5) will maintain their current benefit level even if they were not able to have a preventive dental visit in the current year.
